1. Scope of This Notice
This California Privacy Notice explains how Apos US Management, Inc. d/b/a AposHealth collects, uses, discloses, keeps, sells, or shares personal information about California residents. In this Notice, “we,” “us,” and “our” mean Apos US Management. This Notice also explains the privacy rights California residents have under the California Consumer Privacy Act, as changed by the California Privacy Rights Act. Together, these laws are called the “CCPA.”
2. When This Notice Applies
This Notice applies when California residents contact us or use our services. This may include when you visit our websites, fill out online forms, ask for information, contact us, ask about services, receive services, join marketing or education activities, work with us as a business contact, or interact with us online or offline.
Some information is not covered by the CCPA. For example, the CCPA usually does not cover health information protected by HIPAA, medical information protected by California medical privacy law, or other information that the law excludes. When another law applies, we follow that law and any notice or permission form that applies.
3. Sensitive, Personal Information You May Provide to Us
Personal information means information that can identify you. This may include your first name or first initial and last name, together with information such as your Social Security number, medical information, health insurance information, or appointment information. Personal information does not include public information that is lawfully available from government records.
Medical information means information that identifies a person and relates to that person’s medical history, medical care, diagnosis, or treatment.
Health insurance information means information such as your insurance policy number, member ID number, other ID number used by your health insurer, or information in your insurance application, claims, or appeals records.
4. Purposes for Collecting, Using, and Disclosing Personal Information
We collect, use, and disclose the personal information you give us for these reasons:
- To answer questions, schedule appointments, communicate with patients or people who may become patients, and provide support.
- To check eligibility, coordinate services, process payments, and manage our relationships with providers and payors.
- To send service, education, information, and marketing messages, when allowed by law.
- To keep our systems safe, stop fraud, and protect rights and property.
- To follow laws, rules, legal processes, and government requests.
5. Categories of Recipients to Whom We Disclose Personal Information
We may disclose personal information to these groups when needed for the reasons in this Notice:
- Affiliates and subsidiaries.
- Service providers and contractors, such as hosting, cloud, IT, customer support, and call center vendors.
- Healthcare providers, clinical service providers, payors, insurers, employers, brokers, and others involved in arranging or managing requested services, when this applies.
- Professional advisors, including attorneys, auditors, consultants, and insurers.
- Government agencies, regulators, courts, and people involved in legal matters, when the law allows or requires it.
- Other people or organizations when you tell us to share the information or give us permission.
6. Sale or Sharing of Personal Information
We do not sell personal information for money. We also do not sell personal information as the CCPA defines “sell.” If we share personal information for cross-context behavioral advertising, we will give you a way to opt out, as required by law.
7. Sensitive Personal Information
We may collect sensitive personal information when you give it to us. This may include health information, insurance information, government ID numbers, payment information, or exact location information if we ever collect it.
We use and disclose sensitive personal information only when the CCPA allows it. For example, we may use it to provide requested services, check quality or safety, prevent fraud, protect our systems, process payments, follow the law, or for other allowed reasons. If we use or disclose sensitive personal information for a reason that gives you a right to limit under the CCPA, we will give you a way to make that request.
8. De-Identified and Aggregated Information
We may use information that no longer identifies you. We may use this kind of information for reports, research, analytics, service improvement, and other lawful reasons. If we keep de-identified information, we will not try to identify you from it unless the law allows it. We will also require recipients by contract to follow the legal limits that apply to this information.
9. Retention of Personal Information
We keep personal information for as long as we need it for the reasons described in this Notice, for another reason we disclosed to you, or for a related reason allowed by law. When we decide how long to keep information, we look at legal rules, contracts, possible disputes, security needs, and the reason we collected the information.
10. Your California Privacy Rights
California residents have these privacy rights, unless an exception applies:
- Right to know/access: You may ask what personal information we collected about you. You may also ask where it came from, why we collected it, who we shared it with, and whether we sold or shared it.
- Right to delete: You may ask us to delete personal information we collected from you. We may keep some information when the law allows it, such as to finish a transaction, provide a service, protect security, or follow the law.
- Right to correct: You may ask us to fix personal information about you that is wrong, based on the type of information and why we use it.
- Right to opt out of sale or sharing: You may ask us not to sell or share your personal information. This includes sharing for cross-context behavioral advertising, which means targeted ads based on activity across different websites, apps, or services.
- Right to limit sensitive personal information: When this right applies, you may ask us to limit how we use or disclose sensitive personal information.
- Right to portability: You may ask for a copy of your personal information in a portable format that is easy to use, when technically possible.
- Right to non-discrimination and non-retaliation: We will not treat you unfairly because you use your CCPA rights. We may offer a different price or service level only if the law allows it.

12. Verification of Requests
Before we respond to some requests, we must confirm your identity. The information we ask for will depend on your request, the type of information involved, and our relationship with you. We will only use the information you give us to check your identity, unless you give it to us for another reason.
If we cannot confirm your identity or your right to make the request, we may deny the request or ask for more information. We will respond within the time required by law. If we need more time, we will tell you as required by the CCPA.
13. Authorized Agents
You may choose another person or business to make a CCPA request for you. This is called an authorized agent. We may ask the agent to show written permission or a power of attorney. We may also ask you to confirm your identity and confirm that the agent may act for you, unless an exception applies.
14. Financial Incentives
We do not currently offer money, discounts, rewards, or different service levels in exchange for collecting, keeping, selling, or sharing personal information. If we offer a program like this in the future, we will give the notice required by the CCPA and get any required consent before you join.
15. California Shine the Light
California Civil Code section 1798.83 lets California residents ask whether we shared personal information with other companies for their own direct marketing during the last calendar year, if this applies. To make this request, contact us using the methods listed in this Notice.
